Wellness Program

IF an employee's position is covered under the DPEA, DPESPA, or CMA collective bargaining agreements (i.e. contracts) AND the employee is covered under a D62 medical insurance plan, THEN the employee is REQUIRED to complete the annual wellness screening.

IF an employee's position does NOT fall under a collective bargaining agreement OR the employee WAIVES D62 medical insurance (regardless of whether their position is union or non-union), THEN wellness screening is encouraged but NOT required. 

Exempt employees include:

• Administrators
• 12 month non-union employees
• 10 month non-union employees
• Permanent substitutes
• Day to day substitutes
• Part-time employees who are not eligible for benefits
• Employees who waive coverage (regardless or whether their position is union or non-union)

There are 3 options employees can choose from:

Option 1: 

Participate in the district sponsored wellness screening event (dates, locations and registration information will be shared in September 2025). These events are scheduled by the district and run by Empower Health Services. The event typically takes place from the end of September through the middle of October.

Option 2:

Have your physician complete the Wellness Alternative Screening form.  The form can be completed and submitted anytime between 1/1/25 and 4:00 p.m. on 12/12/25.

• Hover over the word Portals in the menu bar at the top of the home screen on your desktop/laptop or click on the 3 ellipses next to the word Menu on the home page on your iPad
• Click on Staff Resource Center
• Enter your username and password (this is the same as your computer login).  If you have any issues with logging in please email service@d62.org for assistance.
• The form is in the box with the heading Employee Forms

Option 3:

Empower Health Services offers employees a convenient Individual Screening Program with LabCorp.  Participants register online and go to a LabCorp facility at a time convenient for them to complete the screening.  It is the same 42-Component blood chemistry profile that Empowers Health Services provides at their onsite visits in District 62.  There is no cost to employees and insured spouses who participate in the District 62 BCO or HMO plans.  Flu vaccines are NOT available with the LabCorp option. 

To sign up for a LabCorp individuals appointment (available immediately):

Go to empower.health

• Returning Participants:  If you previously created an account, sign in under “Welcome Back!”
• New Participants:  Complete “New User” fields and enter client code: D62EHS  
  • A verification email will be sent.  Verify your email address to activate your account
• On the dashboard, click the blue “Register” box
• Click the purple “Register” circle to select the individual LabCorp program.  Registration for this program will close on 12/11/25 and appointments must be completed by 12/12/25.
• Complete the Empower Health AssessmentTM by clicking “Begin Assessment” when prompted during registration or from the dashboard.
• Once the registration steps are completed, you will receive a confirmation email containing the next steps.  You must wait until you receive the email before visiting the lab.
• Your confirmation email will include your COR order number.  Please bring this number to your appointment.  Empower recommends printing your LabCorp Requisition to bring to the lab.  You can access your requisition and lab information through your personal empower.health portal.
• After your lab visit, you can expect to receive an email letting you know that your screening results are ready to view online approximately 2-3 days from your lab visit.

The wellness screening is for the calendar year.  Therefore, the 2025 wellness screening is for the time period of 1/1/25 - 12/12/25.  This is the same time frame that has been in existence for years.

Employees may complete the wellness screening any time between 1/1/25 through 12/12/25.  The deadline to turn in the Wellness Alternative Qualification form is 12/12/25 at 4:00 p.m.  This is a firm deadline and no exceptions will be provided.

No, BCBS does NOT require a waiting period between wellness exams so you do NOT have to base your 2025 wellness clearance appointment on your 2024 medical visit date.

Failure to meet the 4:00 p.m. deadline on December 12, 2025 will result in a non-participation insurance premium fee of $1200 ($100 per month for the months of January 2025 through December 2025), per the collective bargaining agreements (i.e. union contracts).  This fee will be collected via payroll deductions from January 15, 2026 through June 30, 2026 ($100 per paycheck over 12 pay periods = $1200).  Refer to your collective bargaining agreement (contract) for more information.

Spouses covered under the employee’s D62 medical plan are eligible to participate in the district sponsored wellness screening events at no cost.  Children are NOT eligible to participate.
